Above are two (2) *Applications that you must return, the Holistic Festival application and contract.  Please complete them both and return with your payment.  We will not reserve booth space until payment is received.  Your envelope should include 3 things:  Contract, Application and Check (made out to Sybil Vaughn), also a Lecture Application if desired.
